About the Role
The role of a facilitator is to empower individuals in making informed choices about their lives. This involves working with them to develop programs that cater to their needs and interests, within the context of a person-centred model of service delivery.
The facilitator will work closely with service users to identify their goals and requirements, and provide support in accessing community resources to meet those needs.
This includes developing and delivering programs that promote independent living, personal development, and occupational and leisure activities, all within the framework of a quality of life model.
Responsibilities include:
* Facilitating Person Centred Planning sessions to ensure individualised support plans are in place.
* Maintaining accurate records of service user interactions and progress.
* Developing and implementing programmes to meet the diverse needs of service users, including independent living, personal development, and occupational and leisure activities.
* Providing support and guidance to service users in accessing community resources, such as education and employment opportunities.
