Job Description
We are seeking a dedicated and compassionate Support Worker to join our interdisciplinary team. The successful candidate will work closely with children and their families who attend our service, providing a high-quality care experience.
The Support Worker will report directly to the Children's Disability Network Manager and will be part of an integrated team delivering services to meet the diverse needs of our clients.
Key responsibilities include:
* Assisting with review of clinical reports from private providers and documenting outcomes and recommendations in a standardized template under supervision.
* Transferring or referring clients to other services as appropriate, under the guidance of the Children's Disability Network Manager and clinical team.
* Supporting the discharge of clients from our service, working closely with the Children's Disability Network Manager and clinical team.
* Conducting file reviews and preparing clinical note summaries for reference purposes.
* Participating in team meetings and reporting on matters affecting service delivery to the Children's Disability Network Manager.
The ideal candidate will possess a strong understanding of person-centered care and rights-based approaches, with experience supporting children or individuals with disabilities. A relevant QQI Level 5 Award in Health Care Support / Social Care or equivalent is essential.
Required Skills and Qualifications
To succeed in this role, you must have:
* A recognized qualification in healthcare support or social care (QQI Level 5 Award).
* Experience working with children or individuals with disabilities.
* Excellent communication and interpersonal skills.
* Able to maintain confidentiality and adhere to data protection guidelines.
