Programme Facilitator Role Overview
We are seeking a highly motivated and experienced Programme Facilitator to join our team. As a key member of the team, you will be responsible for facilitating service users in their choices, developing programmes within the context of a person-centred model of service delivery.
Key Responsibilities:
* Facilitate service users in their choices and develop programmes within a person-centred model of service delivery.
* Develop and deliver person-centred programmes to meet the needs of service users as identified through Person Centred Planning.
* Support individuals and groups in accessing their community to meet identified needs by facilitating activities and programmes.
* Liaise with relevant team members to ensure that service users receive the highest standard of service.
Required Skills and Qualifications:
* A minimum of 1 year's experience working in a relevant setting with experience of facilitating and implementing person-centred plans.
* Experience in the design, development, and provision of programmes and activities to meet the needs, choices, and abilities of service users.
* A full driving licence is essential; this role requires travel.
* A minimum QQI Level 5 in Social Care or equivalent.
