Staff Nurse Rehabilitation Service Based at Blarney Rehabilitation Unit or any buildings on the Complex of the Mercy University Hospital. Permanent Contract, Full time Hours. Details of Blarney Rehabilitation Unit Service: Blarney Rehabilitation Unit is a newly developed purpose-built post-acute care rehabilitation unit. It is made up of 50 beds located off site from the MUH. It remains under the governance of the MUH. The Rehabilitation Services program delivers specialist post-acute care, focusing on patient-centered reablement to help patients achieve optimal functionality and independence following illness or injury. This service emphasizes seamless transitions of care between hospital and home, integrating multidisciplinary expertise to support patient recovery. Responsibilities Include: Conduct comprehensive nursing assessments of patients physical, psychosocial, and functional status. Develop individualised, rehabilitation-focused nursing care plans in collaboration with the patient and multidisciplinary team. Regularly review and revise care plans based on patient progress and changing needs. Implement evidence-based interventions to promote mobility, strength, and independence (e.g., assisted transfers, therapeutic exercises). Safely administer prescribed medications and therapeutic treatments. Monitor and document patient responses to medications and interventions, escalating any concerns. Provide tailored education on the rehabilitation process, safe home practices, and self-management strategies. Participate in regular multidisciplinary team meetings with physiotherapists, OTs, physicians, social workers, and others. Maintain accurate, timely patient records in accordance with NMBI and hospital standards. Contribute to discharge planning by liaising with community nursing, primary care, and other support services. Ensure referral forms, equipment requisitions, and follow-up appointments are completed Adhere to the NMBI Code of Conduct and Mercy University Hospital policies. Engage in ongoing professional development and reflective practice. Eligibility Criteria: Candidates must be registered, or be eligible for registration, in the General Nurse Division of the Register of Nurses kept by the Nursing & Midwifery Board of Ireland (NMBI / Bord Altranais agus Cnimhseachais na hireann). Demonstrated commitment to continuing professional development and evidence of ongoing learning. Strong interpersonal, communication, and organizational skills, with a patient-centred approach. Ability to work effectively both independently and as part of a multidisciplinary team. Experience or interest in rehabilitation nursing and managing patients with complex recovery needs. Proficiency in relevant computer applications for patient care and documentation. A post-registration qualification or training in rehabilitation nursing, gerontology, or a related field is desirable. Closing date for applications is 1pm on Friday, 24th October 2025.
