Reporting to the Personal Planning Lead,the Personal Planning Coordinator will support the development and implementation of collaborative high-quality personal planning practices across services in Fingal and Meath.
Key responsibilities of the role include: Develop and deliver Prosper Groups personal planning process in line with national disability policy, best practice, and Prospers core values.
Work collaboratively with the Personal Planning Lead, managers, frontline staff, and key stakeholders to support a high-quality and consistent personal planning approach.
Support and guide frontline staff to implement outcome-focused, evidence-based personal planning, including goal setting and review.
Build capacity across services by educating stakeholders and supporting managers and staff through training, mentoring, guided practice, and shadowing.
Act as a role model for person-centred, rights-based practice, supporting effective personal plans, active Circles of Support, community inclusion, and improved quality of life outcomes.
Required Qualifications & Experience: Level 8 or higher qualification in social care, disability, health, a related area or equivalent professional registration or experience.
Minimum of 2 years experience in personal planning within a social care or intellectual disability setting.
Strong understanding of person-centred planning and human rightsbased approaches.
Ability to work on own initiative with a can-do attitude, demonstrating autonomy and professionalism.
Excellent communication, collaboration, and facilitation skills.
Full, clean driving licence and willingness to travel as required.
CORU registration is required, or actively in progress for applicants who are not yet fully registered.
While it is envisaged that the role will be primarily based in Fingal, the Personal Planning Coordinator will be required to travel between Meath and Fingal as needed to support collaboration, consistency, and service development.
This is a 39-hour-per-week post.
The salary is based on Prospers Social Care Worker grade, with progression to Prospers Grade V.
