About the Role
This key administrative and coordination position provides critical support to adults with disabilities, empowering them to live independently and achieve their full potential.
* Conduct thorough assessments to identify individual needs and develop tailored case management plans.
* Coordinate services and serve as the primary liaison between individuals, families, and healthcare providers.
* Manage service requests, resources, and documentation in accordance with HSE and HIQA standards.
The successful candidate will collaborate closely with service users, families, and multidisciplinary teams to ensure effective service planning, coordination, and delivery.
Essential Skills and Qualifications
* Demonstrated experience in disability services or related health/social care settings.
* Proven expertise in case management, service coordination, or placement management.
* Able to work effectively as part of a multidisciplinary team.
