Disabilities Case Manager Roles and Responsibilities Work as part of the disability service team with special a remit for a particular service Develop relationships and structures with the relevant stakeholders responsible for provision and delivery of services to ensure maximum service provision As required appraise and develop service options / proposals for individual clients who present as requiring service in conjunction with the client and / or family member / advocate, relevant professionals, agencies and management Develop and present a costed business case for provision of services to individual clients as deemed necessary validate proposals for service and approve funding to ensure within the providers annual allocations. Work as part of the team to monitor and ensure governance of non-statutory sector service arrangements. Convene, coordinate and / or attend multi /trans disciplinary case management reviews as required Work in partnership with local HSE community teams to ensure coordinated delivery of services. Maintain appropriate, up to date written and electronic records and activity data in accordance with professional service standard and provide reports and data as required and manage caseload effectively. Ensure that all administrative systems are in accordance with guidelines and to the highest standards Efficiently and effectively manage and monitor budgets / expenditure and resources as may be assigned, and to prepare financial management reports as may be required Adhere to Policies with regard to procurement of services Comply with all HSE policies and procedures Be responsible for the provision of statistics as requested Provide such reports as may be necessary in relation to complaints, parliamentary questions and representations and any data requests issued through the IHA/ REO offices.
