Programme Facilitator
We are an independent voluntary organisation providing services and support for people with disabilities and their families for over 70 years. Our work positively impacts the lives of more than 12,000 people with disabilities, equipping them with confidence, skills, and self-belief to achieve their expressed goals.
About the Role:
* The post holder will facilitate service users in their choices, developing programmes within a person-centred model of service delivery.
* Programmes may include independent living, personal development and occupational and leisure activities, within the context of a quality-of-life model.
Key Responsibilities:
1. To be a key worker to service users facilitating Person Centred Planning.
2. To identify goals and supports required by individual service users and maintain records as needed.
3. To develop and deliver person centred programmes meeting service user needs through Person Centred Planning and maintain records.
4. To support individuals and groups accessing their community to meet identified needs by facilitating activities and programmes.
Requirements:
* A full driving licence is essential; this role requires travel.
* Minimum QQI Level 5 in Social Care or equivalent.
* A third level graduate in Health & Social Care or equivalent is desirable.
