Age Related Care Clinical Nurse Specialist
The Brothers of Charity Services Ireland, Limerick Region is seeking a skilled Clinical Nurse Specialist to provide exceptional care for individuals with age-related needs.
Job Description:
As a Clinical Nurse Specialist in Age Related Care, you will be responsible for delivering high-quality patient care, collaborating with multidisciplinary teams, and providing education to patients, families, and healthcare professionals.
Key Responsibilities:
* Provide comprehensive assessments, develop individualized care plans, and implement evidence-based interventions.
* Collaborate with the healthcare team to ensure seamless transitions, coordinate care, and promote continuity.
* Design and deliver educational programs for patients, families, and healthcare professionals on age-related care.
* Contribute to quality improvement initiatives, research, and publication.
Required Skills and Qualifications:
To be successful in this role, you must possess:
* A degree in Nursing or relevant field.
* Registration as a Registered Nurse Intellectual Disability (RNID) with An Bord Altranais agus Cnimhseachais na hireann (Nursing and Midwifery Board of Ireland).
* Formal recognized post-registration education in Gerontology at level 8 or above on the Quality and Qualifications Ireland (QQI) National Framework of Qualifications framework.
* Minimum 5 years post-registration experience in the division of the register in which the applicant is currently practicing.
* Minimum 2 years experience in the specialist area of caring for older individuals with intellectual disabilities.
