Clinical Nurse Specialist (CNS) / Acute Hospital Liaison Nurse (Intellectual Disability)
Horizons is seeking a highly motivated and enthusiastic Acute Liaison Nurse for the role of the Acute Care Liaison Nurse - ID (CNSp). The role involves supporting the implementation of an integrated care model with Horizons and acute services, enhancing healthcare experiences for individuals supported by Horizons and optimizing service delivery.
The Acute Hospital Liaison – I.D. will provide quality and safe healthcare for people with intellectual disabilities and support adults with intellectual disabilities and their families when accessing acute services. The post-holder will offer expertise and specialist advice on the needs of this population, assist in local educational programs, and collaborate with staff and multidisciplinary teams in both Horizons and acute services to improve service user experiences.
Minimum Requirements
1. Registration and Qualifications: Registered nurse/midwife on the active Register of Nurses and Midwives held by NMBI, or eligible for registration. Registration in the relevant division(s) of the NMBI register is required. In exceptional cases, registration in another division may be considered.
2. Experience: At least 1 year of post-registration full-time experience, preferably in the division of Intellectual Disability, or equivalent experience.
3. Specialist Area Experience: Minimum of 1 year in a specialist care area, or supported to attain this experience if not already possessed.
4. Educational Qualifications: Completion of a postgraduate program (Level 9 or higher on the NFQ, or equivalent ECTS credits) relevant to the role, verified by an education provider or supported by evidence of credits earned.
5. Professional Development: Demonstrated or supported to undertake ongoing CPD relevant to the role.
6. Competence: Ability to practice safely and effectively within scope of practice.
7. Additional Requirements: Good character, health suitable for the role, a full clean driver’s license, and access to own vehicle.

* Maintain annual registration with NMBI and confirm registration annually with the HSE via the PSAC.
