Consultant / GP - UPMC Swords Outreach Centre

Location: SSC Sports Medicine

Overview of role:

UPMC is establishing a dedicated Lifestyle Medicine service focused on prevention, behaviour change, and chronic disease risk reduction. We are seeking a GP or Consultant with a specialist interest in Lifestyle Medicine to lead and deliver evidence based, patient centred preventive care at our new clinic in Swords.
The successful candidate will champion lifestyle based interventions across UPMC sites, work collaboratively within multidisciplinary teams (including cardiology, nutrition, psychology and physiotherapy), and contribute to the strategic growth of UPMC's Lifestyle Medicine services in Ireland. Responsibilities include clinical care, programme development, outreach, and operational input to ensure service excellence.

Primary Duties and Responsibilities:

Key Objectives
Integrate Lifestyle Medicine into routine patient care and UPMC
clinical pathways.
Promote preventive care and empower patients to manage
cardiovascular, metabolic, and mental health risks.
Develop clinic‑based and community‑based programmes focused on
behaviour change and sustainable lifestyle improvement.
Support audit, quality improvement, and outcome-driven service
development.

Core
Responsibilities

Clinical Care
& Lifestyle Assessment

Deliver comprehensive lifestyle-oriented clinical assessments
including history, examinations, functional measures, and behavioural risk
profiling.
Provide personalised intervention plans covering physical activity,
nutrition, stress management, sleep hygiene, smoking cessation, and weight
optimisation.
Lead programmes such as cardiac rehabilitation, weight management,
smoking cessation, and stress reduction.

Diagnostics,
Risk Reduction & Chronic Disease Prevention
Order and interpret investigations (bloods, ECG, risk scores,
functional tests) aligned with preventive care pathways.
Manage early-stage or lifestyle-responsive conditions such as
hypertension, dyslipidaemia, metabolic syndrome, obesity, and
stress-related symptoms.
Ensure safe follow-up, action plans, and clear communication of
results.

Care Pathways
& Multidisciplinary Collaboration
Coordinate care with relevant UPMC or other specialists including
cardiology, endocrinology, nutrition, psychology, physiotherapy, and
sports & exercise medicine.
Participate in MDT meetings and contribute to pathway design and
continuous refinement.
Support the development of structured referral and feedback loops.

Outreach,
Education & Community Engagement
Deliver educational talks, workplace wellbeing engagements, and
community outreach to promote lifestyle-based health solutions.
Strengthen relationships with GPs, insurers, employers, and
community partners.

Governance,
Audit & Outcomes
Maintain accurate clinical records and utilise UPMC digital systems
for outcomes and data reporting.
Support research, audit cycles, and quality improvement
initiatives.
Ensure compliance with Irish guidelines, UPMC clinical governance,
and data protection standards.

Qualifications & Experience:

Registration: Irish Medical Council (IMC) – Specialist Division
(General Practice) or Specialist Register.
Membership: Member of the Irish Society of Lifestyle Medicine or
other.
Experience: Minimum 3 years post‑CSCST clinical experience with
exposure to preventive medicine, chronic disease management, and health
screening.
Lifestyle Medicine Expertise: Demonstrated experience delivering or
leading programmes related to physical activity, nutrition, stress,
smoking cessation, or weight management.
Multidisciplinary Collaboration: Proven ability to work with
integrated care teams.
Communication & Digital Skills: Strong verbal, written, and IT
capability for patient engagement, data management, and team coordination.
Outreach: Experience engaging with external healthcare providers,
community organisations, or insurers.

Education & Advocacy: Skilled in public engagement, CPD
contributions, and promoting lifestyle-oriented health solutions.
Collaboration: Strong teamwork skills across multidisciplinary
settings.
