Dietitians work with their multidisciplinary team members to deliver dietetic services. These consist of one to one services either online or in person, on line or in person group work, home visits and resource development. There are many initiatives including involvement in Healthy Food Made Easy (HFME), Baby Food Made easy (BFME), roll out of Nutrition Care Process (NCP) clinical audit and student training.There are three Integrated Care Programme for Chronic Disease (ICPCD) Hubs across the area (Aligned to the Mater, Connolly and Beaumont hospitals). Senior Dietitians in the ICPCD Hubs work as part of the multidisciplinary care team in delivering one to one clinical care and Self-Management education and Support (DSMES) to adults with Type 2 Diabetes.
