Job Title: Client Care Coordinator
We are seeking a highly motivated and experienced Client Care Coordinator to manage care services for clients in South Dublin and Wicklow.
* This role involves coordinating and managing care services, liaising with clients, their families, and care professionals to ensure the highest level of care is provided.
* The ideal candidate will have proven experience in a similar role in the healthcare industry, excellent communication and interpersonal skills, strong organisational and time-management skills, ability to work well under pressure and in a fast-paced environment, and good IT skills, including proficiency in Microsoft Office.
Key Responsibilities:
* Coordinating and managing care services for clients in South Dublin and Wicklow.
* Liaising with clients, their families, and care professionals to ensure the highest level of care is provided.
* Developing and implementing effective care plans.
* Scheduling and managing staff rotas to ensure adequate coverage.
* Maintaining accurate care records and responding to client queries in a timely and professional manner.
* Conducting regular client reviews and updating care plans as necessary.
* Providing support and guidance to care professionals.
* Ensuring all care services are delivered in accordance with company policies and procedures.
