Job Description
As a key member of our team, you will be responsible for facilitating person-centered planning and supporting individuals in achieving their goals.
Reporting to the Community Service Manager through the Programmes Supervisor, you will work closely with service users to identify their needs and develop programmes that meet those requirements.
With a focus on quality-of-life, you will facilitate independent living, personal development, and occupational and leisure activities that promote community integration and independence.
Key Responsibilities:
* Facilitate person-centered planning with service users to identify goals and supports required
* Develop and deliver person-centered programmes to meet the needs of service users
* Support individuals and groups in accessing their community to meet identified needs
* Liaise with external agencies and families to ensure seamless support
Required Skills and Qualifications:
* Full driving license and own means of transport
* Minimum QQI Level 5 in Social Care or equivalent desirable
* Experience supporting adults with intellectual disabilities and people enduring mental health issues
* Irish Sign Language skills highly desirable
