Project Worker, Housing First, South East
Contract: Full Time, Permanent Contract
Location: South East
The Project Worker will be responsible for supporting people prior to, during, and following their move through case management and key working. The role involves building clients' capacity to meet their goals, address their support needs, and sustain their housing. Responsibilities include processing referrals, conducting assessments, care/support planning and implementation, engaging with service users, supporting move-on plans, managing maintenance issues, and participating in supervision.
What You Will Do
1. Work to the principles and fidelity of housing first.
2. Operate within the framework of the Housing First Intensive Case Management Service objectives.
3. Provide a safe, welcoming environment for clients, respecting consumer choice and self-determination.
4. Establish trusting relationships with clients, especially those living on the streets or in temporary accommodation, to facilitate their move to stability.
5. Undertake assessments and support planning as the lead, using appropriate systems and tools.
6. Develop methods and relationships to support clients with housing, welfare rights, and access to move-on accommodation.
7. Assist tenants in creating and maintaining a home, following tenancy indicators.
8. Build and maintain a portfolio of accommodation providers across social housing, homeless sector, and private rentals.
9. Support clients at appointments with other services.
10. Work as part of a multidisciplinary team and collaborate with other agencies.
11. Support tenants in life skills, rent payments, reporting repairs, and being good neighbors.
12. Provide advice, information, and guidance to prevent or respond to homelessness.
13. Assist tenants in integrating into the community and accessing support services.
14. Support tenants in developing life skills and seeking education, training, or employment opportunities.
15. Maintain a caseload of clients and keep casework records, logs, and assessments updated.
16. Input data into the Focus Ireland database and PASS.
17. Collaborate with internal teams and external agencies as appropriate.
18. Participate in supervision meetings.
19. Implement policies and contribute to their development.
20. Perform other duties as assigned by the Project Leader.
21. Ensure vigilance regarding health, safety, and welfare risks, reporting concerns accordingly.
What You Will Need
* Degree in social care, youth work, or related discipline.
* Experience working with homeless individuals.
* Experience with clients having dual diagnoses.
* Teamworking experience.
* Report writing skills.
* IT proficiency.
* Experience liaising with other agencies.
* Strong communication skills.
* Knowledge of homelessness issues.
* Ability to plan and implement programs.
* Flexibility.
* Full driver’s license.
