Job Summary
As a Community Leadership and Support Specialist, you will play a key role in ensuring the success of our community. This involves guiding, monitoring, and leading frontline residential staff teams to achieve goals and objectives.
Key Responsibilities:
* Oversee day-to-day activities of frontline residential staff teams within designated centres.
* Provide leadership, coaching, and supervision to staff, promoting a positive workplace culture that provides learning and growth for all staff.
* Support Person in Charge (PIC) in developing an evidence-based culture by ensuring service information and action plans are updated, accurate, and completed.
* Contribute to the development of strategies to continuously improve the community and meet identified actions.
* Participate in working groups/teams as appropriate to advance the interests of residents.
Person-Centred Support for Living:
* Ensure residents' assessment of requirements is updated based on their specific needs and goals to enhance their quality of life.
* Support the development and implementation of residents' Support Plans based on their specific needs, choices, and preferences for enhancing their quality of life.
* Maintain complete and accurate records for each resident, including personal needs assessments, risk assessments, incidents, file notes, communication plans, behavioural management support, health action plans, medication management, and financial records.
Staff Coaching and Development:
* Lead the implementation of processes at house level and monitor practice to ensure full embedding of policies, procedures, and work methods.
* Support recruitment and selection from onboarding, induction, training, and probation for frontline staff.
* Identify training needs of the staff team, deliver training to maximize their potential, evaluate training impact, and contribute to the development of improvements.
* Facilitate the development of an environment conducive to promoting 'reflective practice' to ensure care delivered to residents is of the highest standard.
