Reporting to the Community Service Manager through the Programmes Supervisor, the post holder will facilitate service users in their choices, developing programmes within the context of a person-centred model of service delivery. The programmes may include independent living, personal development, and occupational and leisure activities, within the context of a quality of life model.
Minimum Qualifications and Experience
* QQI Level 5 in Social Care or equivalent (desirable)
* Experience supporting adults with intellectual disabilities and mental health issues
* Fluency in Irish (highly desirable)
* Understanding of Irish Sign Language (highly desirable)
* Programme facilitation skills and client handling experience (advantages)
Job Duties & Responsibilities
* Develop and deliver person-centred programmes based on Person Centred Planning; maintain records
* Act as a key worker for service users, facilitating goal setting and support planning; maintain records
* Support individuals and groups in accessing community resources by facilitating activities and programmes
* Adhere to all rules, regulations, policies, and procedures of RehabCare services and the Rehab Group
Additional requirements
* Good report writing and IT skills
* Programme facilitation skills (advantage)
* Excellent interpersonal skills
* Full driving license (desirable)
