Job Role: Housing First Project Worker
The primary objective of this role is to provide support to individuals who are experiencing homelessness and help them transition into secure, permanent housing. This involves case management and key working to build the individual's capacity to meet their goals and sustain their housing.
* Process referrals, conduct assessments, and implement care/support plans.
* Engage with service users, support them with move-on plans, and manage maintenance issues.
* Participate in practice supervision and collaborate closely with the Team Leader and relevant stakeholders.
Main Responsibilities:
* Exhibit exceptional social care practices as a leader.
* Evaluate the needs of service users in areas such as referral, induction, key working/case management, and move-on, ensuring an effective response from the service.
* Ensure that service users access services/community supports/welfare allowances they are entitled to.
Essential Requirements:
* National Diploma in Applied Social Care Studies awarded by Quality & Qualifications Ireland (QQI) or equivalent qualification.
* A minimum of 2 years' experience in residential social care services, preferably in the homeless sector.
* Leadership experience in social care services, including supervision of social care staff, would be beneficial.
