Care & Case Manager – The Good Shepherd Centre Kilkenny
Location: Kilkenny, Ireland. Project: RSS Service.
Purpose of the Post
To work with the Service Manager and within the authority delegated to the post holder; to assist in the day to day running of the service whilst remaining on the roster.
Job Title and Reporting
Job Title: Case Manager. Reports to: Service Manager. Status: Vacant.
Responsibilities
* Process referrals; conduct assessments; support planning and implementation of care/support plans.
* Engage with tenants; support tenancy sustainment plans; address housing maintenance issues; ensure payment of rent where applicable.
* Play a key role in Case Management, Tenancy Sustainment and Resettlement; collaborate with Local Authority as required.
* Provide out-of-hours on-call support where designated; act on behalf of the Service Manager during periods of leave as agreed.
* Support the elderly, disabled and homeless by facilitating quality and affordable accommodation and appropriate levels of support to maintain tenancies and promote social inclusion.
Environment and Guidance
Tar Isteach Housing aims to support service users in housing and social care, working within the policies and framework of Tar Isteach Housing. The post holder will liaise with Service Manager, Regional Manager and relevant Local Authority, Health Service Executive and other agencies.
Principal Accountabilities & Key Indicators
* Tenant/Service user support: assess holistic needs; refer to appropriate services; inform tenants of rights and responsibilities; promote a positive rent culture; support access to services and welfare entitlements.
* Quality: maintain high standards of care and promote tenants’ rights; align with relevant quality standards and frameworks.
* Communication: work in line with Tar Isteach Housing values and contribute to policy development.
* Policy: participate in team meetings; provide data and reports; maintain data quality and confidentiality.
* Property/Information Management: monitor health and safety; report issues; maintain records and MIS as required; support data protection compliance.
* Case-Management Files: maintain case management records; conduct move-in/move-out and regular reviews; foster multi-agency collaboration.
* Monitoring Resources and Crisis Management: advise on resource needs; use crisis intervention methods; develop risk management plans with service users.
Duties and Responsibilities
* Proactively identify and engage tenants requiring support; assess needs and prioritise support.
* Develop and implement care/support plans; conduct regular key-working sessions; perform risk assessments and management.
* Engage in multi-agency case management and reviews; complete required paperwork and enable advocacy with external agencies.
* Foster relationships with external agencies to maximize supports; provide community support and coordination with Local Authority; operate on-call duty roster.
Administration and Accountability
* Administer day-to-day service aspects per Standard Operating Procedures; maintain high-quality documentation; ensure timely data submission; report issues; monthly supervision with the Service Manager; pursue ongoing training; support use of new technology.
Research, Personal Development and General
* Participate in research, audits and service improvement initiatives; seek tenant feedback for service development.
* Engage in training needs analysis; pursue continuing education; maintain supervision to reflect practice; support new service initiatives and flexible working arrangements as needed.
Eligibility Criteria
* Qualifications: National Diploma in Applied Social Care Studies (QQI) or Diploma in Social Care (QQI) or equivalent qualifications; BA (Ord) in Social Care Practice (Level 7) or Open Training College National Diploma in Applied Social Studies (Disability) or equivalent; and a full and clean driving license.
* Experience: Minimum 2 years in residential social care services (preferably in homeless or supported housing); experience in leadership/supervision; experience with challenging behaviors; change management; recording and information management.
* Post-specific requirements: knowledge of Social Care/Community Work principles; health and safety legislation; data protection; case management; key working; holistic needs assessment; PASS database; NVCI/Therapeutic Crisis Intervention knowledge is advantageous; HIQA standards familiarity.
