Job Summary
We are seeking a dedicated and experienced Care Manager to join our team. The successful candidate will make significant contributions to the achievement of all Supported Living activities, practices, and goals aimed at cultivating a high-performance culture focused on employee empowerment, quality, productivity, and standards.
* Manage field care supervisors and staff assigned to work within your designated territory.
* Attend weekly department meetings and review minutes of previous meetings.
* Prepare agendas and review MDT reports or new referrals, service user reviews, and HSE quarterly reports.
* Review incident/accident reports, making observations and recommendations for improved safety practice and escalating through head office for trend analysis.
* Ensure personal plans are developed prior to case setup.
* A focus on sales within the department.
* Ensure all shifts are covered.
* Provide supervision and support to staff.
* Liaise with external agencies as required and build business relationships.
* Achieve key performance indicators (KPIs) activity targets assigned by the Director of Operations.

Requirements
* Qualification: Level 7/8 degree or working towards degree in Social Studies.
* Knowledge: Knowledge of relevant legislation.
* Experience: Minimum 3 years managerial experience in a similar social care setting.
