Healthcare Direct is a leading provider of therapy services in Ireland, and we're currently seeking a passionate and dedicated Social Worker to join our team in Blarney, Cork on a permanent full-time basis.

Our services integrate seamlessly with Mowlam Healthcare and the HSE, providing tailored care that enhances patient outcomes and with a focus on the highest quality care for aging populations.
With ongoing growth and development in integrated rehabilitation, we are looking to expand our team.
Key Responsibilities: Service Provision: Be responsible for the management of the day-to-day provision of the social work service within a rehabilitation site.
Patient Assessment: Assess patients' mental health, social, and financial needs to create comprehensive care plans.
Care Coordination: Collaborate with multidisciplinary healthcare teams in the smooth execution of care plans, including discharge planning to ensure that, upon discharge, patients have an appropriate level of support for their needs in their families and community.
Attend case conferences and meetings as required.
Resource Referral: Connect patients and families with essential community resources like financial assistance, housing, and mental health services.
Crisis Intervention: Provide immediate and compassionate support during medical crises, including end-of-life care.
Advocacy: Ensure patients rights are respected, advocating for their needs within the healthcare system.
Ideal Candidate: CORU Registration Experience in social work with clinical experience in elderly care preferred (but not essential) Strong written and verbal communication skills Must have own transport, a clean drivers license, and a smart phone Passionate about providing patient-centered care in a dynamic, multidisciplinary environment Support and Development: Healthcare Direct values its team members and supports their professional growth.
