* Our facilitators work in educational, developmental, recreational and occupational areas as appropriate.
Key Responsibilities
* We commit to organisational values of teamwork, dignity, justice, advocacy and quality.
* Key workers facilitate Person Centred Planning processes for service users.
* We identify goals and supports required by individual service users and maintain records.
* Person centred programmes are developed and delivered to meet service user needs, with relevant records maintained.
* Daily duties include health and safety checks, communication with external agencies and families, and general supervision.
* We work as part of a team, ensuring good communication within the team and with external agencies and families.
* Rules and regulations of RehabCare services are observed at all times, adhering to administrative rules, regulations policies and procedures.
* Independence and community integration are promoted in our approach with service users, including liaison with key personnel in pursuit of accessible community activities.
* Health and safety legislation is complied with, maintaining the highest standards of safety, hygiene and housekeeping within the service.
