The Transition Coordinator will work collaboratively with key stakeholders to deliver on the organisation plan to support each person living in its small adult disability residential service to transition from their existing service to established services supporting people with intellectual disabilities and complex needs to realise their potential and live a good life.

Essential Criteria - ideally the successful candidate will possess the following: An appropriate 3rd level qualification in Social Care, or equivalent.
Minimum 5 years post qualification experience Experience of supporting adults with disabilities to transition from congregated to community setting.
Experience in service planning development Experience of implementing a system to monitor outputs and outcomes and the delivery of activities reports.
