We are seeking an experienced Patient Transport Coordinator who can manage the daily bookings from patients, process patient referrals, engage with our partner hospitals and ensure the smooth delivery of our free transport service.
As part of our small team, you will be responsible for:
* Receiving and collating referral information from our partner hospitals
* Processing patient referrals from our partner hospitals, making contact with the patient and taking booking information
* Managing daily patient bookings and inputting information on our CRM database
* Process financial support grant applications where appropriate
* Managing incoming and outgoing patient calls and emails responding to a large volume of incoming calls
* Addressing logistic issues as they arise and ensuring proper procedures and quality standards are adhered to
In addition to these duties, you will also be required to:
* Administer patient feedback surveys when required and collate the findings
* Ensure continuous service improvement through patient feedback and signposting to other services and supports
* Proactively identify opportunities for improvement in our service delivery or operations
* Build and maintain relationships with existing internal and external stakeholders and develop new relationships as required
* Work closely with other teams within the Society to provide a full support service to all referred patients
To be successful in this role, you will need to have:
* A degree or professional qualification in social work, community development or related field (or relevant experience in lieu)
* At least 3 years' experience in service delivery or community support
* An understanding of patients' needs and empathy for those who find themselves in difficult situations
* Ability to work as part of a team and on your own initiative
