Name of Project: Own Front Door Project + Long Term Accommodation (Kilkenny)
Introduction to the Model
The OFD project seeks to develop effective and sustainable Emergency accommodation and services (through housing, as an alternative to hostels and hotels) for families and single people referred by the relevant Local authority. The project worker will assess individuals/families to establish appropriate services and supports, including schools, parenting, and childcare. The main goal is to coordinate access to and exit from suitable housing, healthcare, and social support to end homelessness and promote long-term tenancy sustainability. Some case work will be allocated under our Housing Plus programme to support long-term tenants in crisis and ensure tenancy sustainment.
Good Shepherd Centre Kilkenny works with Mental Health Service Area 5 and partners with Kilkenny, Tipperary, Laois, and Carlow County Councils.
Main Roles and Responsibilities
* Work within the framework of the overall objectives and policies of the Kilkenny Voluntary Housing Association.
* Undertake assessments and support planning with clients, focusing on recovery and life skills to promote tenancy sustainment.
* Support tenants in accessing recovery-focused training, education, and employment opportunities.
* Work as part of a multidisciplinary team and maintain a caseload of clients, providing ongoing support until cases are handed over to appropriate services.
* Support clients with appointments to other services, as appropriate.
* Maintain up-to-date casework records, daily logs, and assessments, including data input on Focus Ireland database and PASS.
* Collaborate with other teams across mental health, addiction, Tusla, schools, and community services, including statutory and voluntary agencies.
* Follow service models and standards of best practice.
* Attend supervision sessions and nominated training.
* Supervise students or volunteers as assigned.
* Work a flexible roster, including evenings and weekends, to meet service objectives.
* Implement and contribute to policies and procedures, and plan, implement, and evaluate daily activities.
* Perform other duties as assigned in consultation with the team leader or manager.
* Ensure health and safety in the workplace, reporting concerns appropriately.
Additional Role Details
Engage with service users to facilitate their housing and recovery goals based on recovery principles of hope, control, and opportunity. Use a strengths-based approach in needs assessments to develop care plans covering mental health, addiction, training, employment, parenting, and housing. Assist clients with navigating local authority processes, monitor private housing options, support participation in community activities, develop life skills, coordinate with support services, facilitate peer support, and promote tenancy sustainment and safety.
Essential Criteria
* Two years post-qualification experience in a related field
* Full clean driving license and access to a car
* Level 7 or above in social care
* Good verbal and written communication skills
