Pharmacist Leadership Role
The position of Pharmacist is a key role within the Pharmacy Clinical Oncology Team, with a focus on providing clinical oncology and aseptic services to enhance pharmaceutical care for patients.
Key Responsibilities:
* Provide clinical oncology and aseptic-based services, including advice to medical and nursing staff.
* Perform all duties in line with Pharmacy Department procedures and protocols, as well as hospital-wide procedures.
* Participate in continuing education and research activities consistent with the post.
* Ensure best practice and compliance with agreed clinical guidelines.
Aseptic Services:
* Responsible for undertaking technical checks and final releases for individually dispensed aseptic products prepared, including adult intravenous additives and cytotoxic chemotherapy.
* Clinically check and validate prescriptions received from wards for aseptic products and chemotherapy.
Clinical Service Delivery:
* Participate in ward and clinical oncology pharmacy services, providing advice to ensure safe and cost-effective prescribing and drug expenditure control.
* Undertake medicines reconciliation of patients admitted to hospital, ensuring continuity of medication and recording relevant information.
Information Technology:
* Provide support to clinical staff on the use of automation/E-prescribing.
* Train NCHDs and other clinicians in the use of E-Prescribing systems.
Requirements:
* Be registered with the Pharmaceutical Society of Ireland.
* Have at least 3 years post-registration hospital experience, preferably specific experience in implementing e-prescribing and oncology pharmacy experience.
