Report to: Team Leader Homecare
Role Overview: The Homecare Project is a customised high‑level support model for mental health service users at risk of self‑neglect, hoarding, or incapacity to maintain an adequate standard. The role supports tenants in the programme, works within the Good Shepherd Centre framework, and collaborates with Carlow/Kilkenny HSE Mental Health Services and Kilkenny Local Authority.
Responsibilities
* Support tenants by accessing recovery focused training, education and employment opportunities.
* Work as part of a multi‑disciplinary team and service.
* Maintain a caseload of clients and offer ongoing support until cases are handed over to appropriate support services.
* Supervise the health care worker’s role and duties in conjunction with the manager.
* Accompany and provide support to clients with appointments to other services where appropriate.
* Keep up to date casework records, daily logs and written assessments.
* Co‑operate with other teams across mental health and community‑based services, especially statutory and voluntary agencies.
* Meet best practice standards and attend all nominated training by line manager.
* Supervise students or volunteers as requested.
* Coordinate and implement the homecare support care plan with the homecare support worker.
* Work a flexible roster over 5 days and occasional evenings.
* Implement existing policies and contribute to the development of new ones.
* Provide an on‑call role on a rostered basis.
* Plan, implement and evaluate daily activities.
* Engage with service users to facilitate attainment of housing and recovery goals, using a strengths‑based approach.
* Conduct needs assessments, assist navigation of local authority processes, and support life‑skills development.
* Facilitate peer support when appropriate and promote service user safety, health and wellbeing.
Essential Criteria
* 2‑year post qualification experience in a related field.
* Level 7 or above in social care.
* Full clean driving licence and access to a car.
* Good verbal and written skills.
* Excellent problem‑solving skills.
