Job Opportunity: Housing Support Worker (Creating Foundations)
Location: CH07 (Dublin South/Kildare/West Wicklow) Reporting to: Assistant Project Leader
Primary Purpose: Empower individuals with mental health challenges in CHO7 to live independently and engage fully in their communities. Collaborate with HSE Adult Mental Health Services to provide support for independent living and ongoing post-settlement assistance.
Key Responsibilities:
* Assess clients referred by HSE mental health services in CHO7.
* Foster respectful relationships to achieve housing and support goals in line with support plans.
* Assist in finding suitable accommodation, support moving, and encourage personal responsibility.
* Provide regular post-settlement support through home visits.
* Conduct assessments, reviews, and support plan development.
* Establish connections with local services and community groups.
* Align with Focus Ireland and Creating Foundations objectives.
* Facilitate the Creating Foundations model within CHO7.
* Work under supervision and attend relevant meetings.
* Comply with Focus Ireland policies and maintain accurate records.
* Communicate updates to Mental Health Services regarding clients.
* Participate in training and development as agreed.
Qualifications and Experience:
* Relevant Third Level qualification in social care or related fields.
* Experience in social care or supported housing projects.
* Experience supporting individuals with mental health diagnoses.
* Familiarity with child safeguarding and risk assessments.
Skills and Attributes:
* Ability to work independently and within a team.
* Flexibility, adaptability, and initiative.
* Full driving license and car access for broad coverage.
* Advocacy for vulnerable client groups.
* Report writing and IT skills.
