**Job Title:** Service Coordinator
About the Role
We are seeking a highly skilled and dedicated individual to join our team as a Service Coordinator. This is an exciting opportunity to make a positive impact on the lives of vulnerable adults and children, supporting individuals with mental ill health, learning disabilities, autism, and dementia.
Key Responsibilities
* Plan, coordinate, and implement safe transitions for new individuals into our service and community.
* Liaise with families and relevant members of multi-disciplinary teams, community groups, and voluntary bodies to promote individual needs.
* Design and deliver individual and group programmes of support to enhance capabilities and independence.
* Coordinate ongoing assessment and review through suitable assessment models.
* Support physical, emotional, and personal needs, upholding principles of respect, privacy, dignity, fulfillment, independence, and choice.
* Ensure medication storage and administration comply with current statutory and legislative procedures.
Requirements
* Minimum 2 years experience in a health and social care setting.
* Commitment to undertaking a professional qualification.
* Possess a valid driving license and access to own vehicle.
Desirable Skills and Qualifications
* Experience in leadership or management roles within health and social care.
* Leadership skills, strong verbal and written communication skills, report writing skills, assessment and evaluation skills, computer skills, risk assessment skills, problem-solving skills, organizational skills, time management skills.
