Key Responsibilities
1. To facilitate Person-Centred Planning for service users, identifying goals and supports required, and maintaining records as necessary.
2. To develop and deliver person centred programmes tailored to meet the needs of service users, ensuring record maintenance.
3. To support individuals and groups in accessing their community by facilitating activities and programmes, and performing other reasonable duties deemed appropriate by the Programmes Supervisor/Community Services Manager.
4. To work independently as a lone worker, liaising with relevant team members to ensure service users receive the highest standard of service, communicating with external agencies and families while maintaining relevant records.
5. To adhere to all rules and regulations of RehabCare services, following Rehab Group administrative rules, regulations policies and procedures at all times.
6. To promote and support independence and community integration, liaising with key community-based personnel to pursue accessible community activities aligned with the service user's choices and aspirations.
7. To maintain the highest standards of safety, hygiene, and good housekeeping within the service, ensuring safe transportation of clients when using transport by adhering to the use of all safety equipment.
8. To engage with service-users in a professional manner, demonstrating dignity and respect, and reporting any concerns to the Programmes Supervisor or Community Services Manager as necessary.
