We are seeking an experienced, compassionate, and dedicated Nurse to oversee the delivery of complex care packages for older persons across County Cork. This position is ideal for a nurse with a background in, homecare, or care of the older person who is passionate about maintaining high clinical standards and supporting regional care teams in delivering exceptional care.As nurse, you will conduct holistic assessments, develop individualised care plans, supervise and support field staff, and ensure compliance with regulatory requirements. You will play a central role in driving service quality, improving outcomes, and ensuring service users receive safe, dignified, person-centred care.Key Responsibilities1. Care Planning & Clinical AssessmentDevelop personalised, realistic, and patient-centred care plans in collaboration with service users, families, and multidisciplinary healthcare professionals.Conduct comprehensive, holistic nursing assessments, identifying needs, risks, and required interventions.Implement clinical strategies that support positive health outcomes and promote independence.2. Leadership & Team SupportProvide clinical leadership to a team of field-based care staff.Foster a culture of accountability, professionalism, and excellence in care delivery.Support care staff with advice, guidance, and clinical decision-making as required.3. Staff Supervision & DevelopmentConduct regular staff supervisions, spot checks, and competency assessments.Provide mentorship, coaching, and ongoing professional support to field-based staff.Participate in staff training, induction, and upskilling activities.4. Quality, Safety & ComplianceMaintain high-quality documentation that meets all regulatory, statutory, and organisational standards.Ensure compliance with policies, procedures, and clinical governance requirements.Monitor and audit care delivery standards, addressing issues swiftly and effectively.5. Performance Monitoring & Service DevelopmentTrack service delivery against KPIs and performance measures.Identify areas for improvement and contribute to developing and enhancing service quality.Participate in service reviews, audits, and clinical governance meetings as required.6. Communication & LiaisonAct as a key point of contact for service users, families, and healthcare professionals.Collaborate effectively with GPs, hospitals, allied health professionals, and internal teams.Ensure timely and accurate communication across care pathways and multidisciplinary teams.Essential CriteriaNursing Registration: In-date registration with the NMBI (Nursing and Midwifery Board of Ireland) with no restrictions.Experience: Minimum 1–2 years post-registration nursing experience.Clinical Skills: Experience in community nursing, homecare, or care of the older person desirable.IT Skills: Competent in clinical systems and comfortable using technology for documentation and communication.Driving Licence: Full clean driving licence (role requires travel across County Cork).EducationBachelor's degree in Nursing or equivalent qualification recognised by NMBI.Job Type: Full-timePay: €40,000.00-€45,000.00 per yearBenefits:Company carFlexitimeOn-site parkingWork Location: In person
