Job Description
We are seeking a skilled and compassionate individual to facilitate service users in their choices, developing programmes within the context of a person-centred model of service delivery.
About the Role
The post holder will work closely with the Community Service Manager through the Programmes Supervisor to support service users in achieving their goals.
The programmes may include independent living, personal development, and occupational and leisure activities, all within a quality-of-life framework.
Key Responsibilities
* Be a key worker to service users, facilitating the Person Centred Planning process
* Identify goals and supports required by individual service users, maintaining records as needed
* Develop and deliver person-centred programmes to meet the needs of service users
* Support individuals and groups in accessing community resources to meet identified needs
Requirements
To be successful in this role, you will require:
* A minimum QQI Level 5 in Social Care or equivalent qualification
* Experience supporting adults with an intellectual disability and people enduring mental health issues
* Irish Sign Language skills are highly desirable
