Responsibilities
 * Promote awareness of HIV/Drugs/Sexual Health through education, information and support to local communities and individuals at risk.
 * Provide outreach services to adults and U18s concerned with drug or alcohol use, working to minimise related harm.
 * Operate as part of a multidisciplinary team that assesses, plans, implements and evaluates the health care needs of service users.
 * Facilitate and maintain links between individuals and health care and other relevant services to improve health and social wellbeing.
Qualifications & Experience
Experience working with addiction, homelessness, mental health or other health care needs of marginalised populations is expected. Demonstrated ability to work collaboratively within multidisciplinary teams is essential.
Service Information
The Outreach Worker operates within the HSE Dublin North East region, which delivers community and hospital care across Dublin, Louth, Meath, Monaghan and parts of Cavan. The outreach programme focuses on minimising drug and alcohol‑related harm and improving the health and social wellbeing of a marginalised population cohort.
