Medical Social Worker Kilkenny Emerald Locums is recruiting on behalf of their client for a Medical Social Worker to join a supportive and professional team in an acute hospital setting based in Kilkenny. This role offers an opportunity to work closely with patients and their families during times of illness, trauma, and transition. You will be part of a multi-disciplinary team delivering essential psychosocial support and helping ensure safe discharge planning and continuity of care post-hospitalisation. Role Details: Location: Kilkenny (hospital-based) Hours: Flexible full-time or part-time (up to 35 hours per week) Contract Length: Ongoing Rate: €30€40 per hour, depending on experience Agency: Emerald Locums Main Responsibilities: Provide professional social work support to patients and their families. Conduct psychosocial assessments and deliver suitable interventions. Support people coping with illness, loss, trauma, or end-of-life care. Take part in discharge planning and work with community services to ensure appropriate follow-up care. Offer crisis support in areas such as elder abuse, homelessness, or safeguarding. Maintain accurate clinical records and documentation. Attend and contribute to MDT meetings, family conferences, and case reviews. Collaborate with a wide range of professionals within and outside the hospital. Contribute to ongoing service development. Requirements: CORU-registered Social Worker. A recognised qualification in Social Work. Minimum of 2 years post-qualification experience in a hospital or healthcare setting. Strong understanding of hospital discharge processes, safeguarding, and patient advocacy. Excellent communication, organisational, and case management skills. Ability to work well in a fast-paced multidisciplinary environment. Desirable Experience: Background in palliative care, emergency department, or mental health social work.
