ResponsibilitiesThe Clinical Nurse Care Coordinator collaborates with clinic team members to provide evidence-based, patient-centered care, helping patients stay up to date on their care plans and related health goals.
This role focuses on high-risk, medically complex patients, those on specific disease registries, patients with chronic disabilities, and those in care transitions, acting as a liaison with families and other agencies to facilitate comprehensive, whole-person care.
The RN stays current on standards and assists with programs and processes.
QualificationsAssociate Degree in Nursing from a CCNE or ACEN accredited program.Current Ohio RN license.CPR/BLS & ACLS certification.Experience with triage procedures in clinics, physician offices, or urgent care settings.Ability to maintain quality, safety, and infection control standards.Knowledge of medication administration procedures.Ability to educate and communicate effectively with patients and families.Strong written and verbal communication skills.High interpersonal skills for handling sensitive situations.Poise, tact, and diplomacy.Analytical and critical thinking skills for data interpretation and problem-solving.Pay RangeUSD $31.11 - USD $46.66 per hour.
