Position Statement
The Case Manager plays a key role in the coordination, management, documentation, and support of the treatment and programming of youth within our care. Of equal importance is the Case Manager’s role as a liaison, developing and maintaining rapport and communication with the county, family, and the youth. This is a full-time, benefit-eligible position scheduled to accommodate individual and case-by-case treatment needs. Work weeks may exceed 40 hours, including occasional weekend hours, with all overtime pre-authorized. The Case Manager must have reliable transportation and may use personal vehicle for work-related travel, which will be reimbursed at the agency’s rate.
Responsibilities
* Collaborate and communicate with county workers, service providers, program staff, and family members to ensure effective information exchange, logistical support, and resolution of concerns, advocating for the youth’s best interests.
* Regularly consult with clinicians to translate treatment goals to families, county, and program.
* Coordinate with internal departments to ensure service and treatment consistency.
* Attend required meetings as directed by the supervisor.
* Maintain accurate records, reports, and documentation per agency and state policies.
* Support families with visitation, home assessments, etc., as needed.
* Collaborate with the program management team for treatment and program planning.
* Represent the organization professionally, embodying its values and treatment philosophy.
* Promote principles of agency-wide initiatives like TCI and PEM models.
* Adhere to all policies, protocols, and procedures.
* Perform other duties as assigned, noting that responsibilities may evolve.

* Exhibit high-level written and verbal communication skills.
* Manage paperwork within established timeframes.
* Maintain professional relationships with staff, youth, parents, and community partners.
* Uphold high ethical standards and punctuality.
* Work independently and collaboratively within a multidisciplinary team.

Required Education and Experience
* Bachelor’s degree in Social Work, behavioral sciences, or related field, or Associate’s degree with 3 years of case management experience.
Essential Job Qualifications
* Ability to lift 25 pounds, sit, squat, crouch, stand for extended periods, and reach overhead.
* Ability to walk stairs multiple times daily.
* Proficiency with Word, Excel, Outlook, Teams.
* Valid NYS driver’s license and insurance.
