This is an exciting opportunity to join a leading charity as a Support Coordinator. As a field-based role, you will travel within the community to provide practical support and coordinate services for older people.
* Provide personalized support plans to address challenges and enhance the older person's quality of life.
* Provide access to essential services while coordinating and enabling older people to access other support services in the community.
* Advocate for older people's needs within health, social care, and welfare services, working to streamline access and enhance service delivery.
* Maintain strong relationships with statutory and non-statutory agencies in the designated area, fostering collaboration and access to a wider range of support services.
Key Responsibilities
Support Coordination:
* Devising personalized support plans to address challenges and enhance the older person's quality of life.
* Providing access to ALONE's services while coordinating and enabling older people to access other services in the community.
* Advocating for older people's needs within health, social care, and welfare services, working to streamline access and enhance service delivery.
* Maintaining records on ALONE's CRM database, capturing essential details to inform service delivery and future strategies.
Requirements:
* A demonstrated passion for improving the lives of older people and commitment to the organization's mission.
* Problem-solving skills, focusing on efficient needs assessment and resource allocation.
* Excellent communication abilities (written and verbal), fostering partnerships and negotiating effectively with diverse stakeholders.
* Proficiency in utilizing technology and data management systems, enhancing service efficiency and reporting accuracy.
* Good organizational and multitasking skills, with a solution-focused approach to complex challenges.
* Keen attention to detail, enabling accurate record-keeping and effective service monitoring.
