Clients in County Clare will benefit from a 'Housing First' approach through our housing team.
Key Responsibilities:
* Deliver a holistic approach to individuals experiencing long-term homelessness as part of the housing team.
* Collaborate with colleagues to support clients in achieving stable lifestyles and well-being.
* Conduct comprehensive assessments and create tailored support plans using evidence-based tools and systems.
* Develop strong relationships with accommodation providers and assist tenants in creating and sustaining independent living arrangements.
* Accompany clients on appointments and maintain a caseload, ensuring continuity of care.
Requirements:
* Degree in social work, youth development, or a related field.
* Knowledge and understanding of homelessness, its causes, and the complexities affecting individuals who are or have been homeless.
* Ability to plan, implement, and evaluate programs, report writing skills, and proficiency in IT systems.
* Flexibility, effective communication, and interpersonal skills.
* Full driver's license and access to a vehicle (desired).
