Job Purpose/SummaryThe Lead Community Facilitator is part of the support team whose function is to meet the practical, physical, and emotional needs of individuals with intellectual disabilities availing of a Day Service provided by Lumen HealthCare based in the Ballyshannon/Bundoran areas but providing support to Service Users and Staff in our services in Letterkenny and Buncrana. Through a key worker system, he/she will also be responsible for the designing of individual programmes of support and co-ordinating the delivery of the support package by a group of Community Facilitators.Key Responsibilities· To ensure that all work within the Day Service is focused on community participation and active citizenship addressing the need for positive outcomes for each individual.· To develop, monitor and review individualised person-centred plans in keeping worth the ethos and principles of New Directions model of day services.· To plan, develop and implement individual and group programmes of support based on the needs of each individual.· To complete assessments on each individual service user identifying the necessary supports they will and the risks in relation to each area of daily living.· To participate in a systematic approach to meeting the holistic needs of the individual service user.· To engage with all relevant stakeholders at a professional level in maintaining a multi-disciplinary approach to achieving the optimum care for each individual using our service.· To address and manage complex needs through implementation of Positive Behaviour Support and an ethos of positive risk taking.· To provide effective leadership, supervision, and management to Community Facilitators and to co-ordinate their input into individual programmes of support in.· To personally contribute towards a happy and therapeutic environment and to the workings of a caring team.· To develop and maintain required documentation in terms of Operations, Health and Safety, Medication Management, Finance.Requirements of the Role· To adhere to and model the company values, behaviours, and competencies at all times.· To participate when required in formal and informal supervision.· To perform all duties of the post in accordance with Lumen HealthCare rules, regulations, and policies.· To perform additional duties of the role and all other required duties as and when necessary.The above-mentioned duties are not exhaustive, and the post holder will be required to perform other duties as and when necessary.
