Intensive Case Manager, Housing First, South West
Location: Cork
Contract: Full Time, 1 year Fixed Term
The Purpose of this role is to work as a part of Housing First Case Management Service to deliver a ‘Housing First’ approach to individuals experiencing long term homelessness in Cork City.
What you will do:

Work in line with the principles and fidelity of the Housing First model and within the objectives, policies, and procedures of the Housing First Intensive Case Management Service.
Provide a safe, welcoming, and supportive environment for clients who are out of home or at risk, respecting choice and self‑determination.
Build and maintain trusting relationships with clients, particularly those experiencing homelessness, supporting progression to stable and independent living.
Complete assessments and develop support plans using Care and Case Management systems and tools (e.g. STAR Outcomes) as the lead worker.
Support clients to access housing, welfare rights, entitlements, and appropriate move‑on accommodation, and build links with housing providers across all sectors.
Assist tenants in creating and sustaining tenancies, including rent payments, property care, reporting repairs, and being a good neighbour, in collaboration with the Property Officer.
Provide advice, information, and guidance, and support clients to access health, social, and community services, including attending appointments where appropriate.
Support tenants to develop life skills and access education, training, and employment opportunities, promoting community integration.
Maintain and manage an active caseload, ensuring accurate and up‑to‑date casework records, daily logs, and database entries (Focus Ireland systems and PASS).
Work collaboratively within a multi‑disciplinary team and liaise effectively with internal teams and external statutory and voluntary agencies.
Participate in supervision, team meetings, training, service planning, and evaluation, and contribute to best practice and service development.
Work agreed hours (39 hours per week), carry out additional duties as required, and uphold health, safety, and welfare standards, reporting any concerns appropriately.

What you will need:

A Third Level qualification at degree level in the area of social care i.e. youth work, social studies, addiction, psychology, etc and/or be a CORU registered Social Care Worker.
Experience of working with customers who are or have been homeless.
Experience of working with customers identified as having a dual diagnosis.
Report writing abilities.
Experience of working and liaising with other agencies.
Full driver’s license (Desirable).
