Position

An exciting opportunity has arisen for an experienced pharmacist to join our South Intermediate Care Team based at Biggart Hospital, Prestwick

Hours

From 1 April 2026, the working week for NHS Agenda for Change staff in Scotland will be reduced. Full-time hours will reduce from 37 to 36 hours (pro rata for part-time staff) without loss of earnings.

What You Will Do

Outline of main tasks, duties and responsibilities of the role

The postholder will be responsible for delivering a highly specialised pharmacy service to patients under the care of the South Intermediate Care Team. This role offers the opportunity to gain experience within a community-based multidisciplinary team focused on preventing hospital admissions and facilitating early discharge.

Key responsibilities include conducting comprehensive medication reviews to support patients in managing their medicines safely and independently at home. The role also involves providing expert pharmaceutical advice and educating patients and caregivers about their medications and health conditions. Close collaboration with a range of health and social care services—including social work, acute hospital departments, General Practice, Community Pharmacy, and other key stakeholders is essential to ensure seamless and effective medicines-related care

Knowledge, Training And/or Experience Required To Do The Job

You should possess sound clinical knowledge, strong IT skills, and excellent communication, interpersonal, and problem-solving skills. The ability to work both autonomously and effectively within a multidisciplinary team is essential.

If you do not currently hold an Independent Prescribing qualification, support will be provided to enable you to complete this.
