Role Summary
Cpl Healthcare, a leading healthcare provider, partners with a non-profit homeless service organization to deliver a 'Housing First' approach in County Clare.

About the Role
* Deliver a Housing First approach to individuals experiencing long-term homelessness.
* Promote stable lifestyles through engaging relationships and personalized support.
* Develop and implement programmes to assist clients in progressing towards independent living.
Key Responsibilities
1. Provide a safe environment for clients and offer basic care and support while respecting consumer choice.
2. Establish trusting relationships with clients who are living on the streets or in temporary accommodation.
3. Undertake assessments and support planning using appropriate tools and systems.
4. Support clients in accessing move-on accommodation and developing methods for sustaining a home.
5. Maintain a portfolio of accommodation providers and accompany clients to appointments with other services.
Requirements/Experience Needed
* Degree in social care, youth work, or related discipline.
* Knowledge and understanding of homelessness and its associated issues.
* Ability to plan and implement programmes effectively.
* Strong communication and flexibility skills.
