Optimizing Patient Care
The CNS plays a pivotal role in delivering high-quality patient care, focusing on Oncology patients requiring SACT. Their expertise encompasses providing specialist nursing care, advice, information, and support to newly diagnosed patients and those undergoing treatment.
Responsibilities include:
* Delivering personalized oncology nursing care, guidance, and support to patients with complex needs.
* Developing and implementing effective education programs for cancer patients and their families.
* Collaborating with the Oncology Consultant Clinic team to coordinate patient care and facilitate seamless transitions.
* Identifying and streamlining referral pathways for cancer patients, ensuring timely access to specialized care.
* Working closely with Oncology Clinical Nurse Managers and Surgical CNSs to enhance treatment outcomes and follow-up care.
Key areas of focus include managing the Nurse Led Oral Anti-Cancer Medication Clinic according to National Cancer Control Program guidelines. Additionally, comprehensive holistic patient assessments will be conducted using Common Terminology Criteria for Adverse Events (CTCAE). The CNS will ensure that patients and families receive accurate information, are well-informed throughout their care journey, and utilize resources efficiently.
Continuous quality improvement is essential, and the CNS will actively review and refine the Oncology CNS service standards. Participation in oncology professional groups and conferences will also be prioritized to stay updated on best practices and emerging trends.
