* Community Nursing Unit, Arus Breffni, Manorhamilton, Co. Leitrim
* St. Patrick's Community Hospital, Carrick-on-Shannon, Co. Leitrim
* Arus Carolan Community Nursing Unit, Mohill, Co. Leitrim
* Ballinamore Community Nursing Unit, Tully, Ballinamore, Co. Leitrim
If you apply and are successful at interview, you will be placed on a panel. Once on the panel, you will be informed about all Health Care Assistant (HCA) posts within Older Persons Services in Sligo & Leitrim. Each time a position arises, all HCAs on the panel will be contacted via email, which will include details such as location, start date, service, caseload, team, and contact details of the Nurse Manager at that site. If you're not interested in a particular role, you can ignore it and wait for a role that suits you.

Reporting Relationship: The Health Care Assistant reports to and works under the supervision of the Clinical Nurse Manager or a designated Officer within each unit, who in turn reports to the Person in Charge.
Purpose of the Post: In line with the person-centred philosophy of the Community Nursing Unit and the developing Primary Care Teams & Health & Social Networks, the post holder will support clients/residents in exercising independence and autonomy. Working with the multidisciplinary team and under supervision of nursing or other professional staff, the Health Care Assistant will assist in delivering health & social care, including activities of daily living, tailored to the individual’s needs and wishes, as outlined in an agreed care plan.
