As a dedicated caregiver, you will play a vital role in delivering exceptional support to vulnerable children and their families who require our services.
Key Responsibilities:
* Assist with the review of clinical reports from private providers and document outcomes and recommendations in a standardised template under the supervision of a senior team member.
* Support the transfer or referral of clients to other services where necessary, under the guidance of a senior team member and clinical professionals.
* Contribute to the discharge process of clients from the service, working closely with a senior team member and clinical professionals.
* Assist with file reviews and preparation of clinical note summaries, ensuring accuracy and attention to detail under the supervision of a senior team member and clinical professionals.
* Participate in the administration and day-to-day operation of the service as needed, including research and preparation of resources for universal family support and strategies.
* Maintain high standards of documentation, adhering to local guidelines, principles of confidentiality, Freedom of Information (FOI), and GDPR Acts.
* Contribute to the development and implementation of information sharing protocols and audit systems.
* Collaborate with colleagues to enhance the ongoing development of the service, aligning with best practice and regulatory objectives.
Requirements:
* QQI Level 5 Award in Health Care Support / Social Care / Special Needs Assisting (or equivalent).
* Experience supporting children or individuals with disabilities.
* Understanding of person-centred care and rights-based approaches.
