Purpose of the Post
Social Inclusion seeks to provide equal access to health supports and services for people from vulnerable groups in the community including Traveller, Roma, Migrant, LGTBQI+, Addiction and Homelessness. It recognises the complexity of needs to said groups and part of the Grade V S.I Liaison Officer is to support, advocate, and develop individual care plans for clients you serve from social inclusion cohort. Links are to be built, maintained and strengthened within relevant local services, as a means of improving the health outcomes of these marginalised groups.
