Care About You is a progressive Irish owned care provider of Elderly Care and Disability services to individuals in their own home.
We are recruiting a Community Nurse to be based in our Shannon office who will provide clinical services including support and guidance to service users in an effective and caring manner and to a high standard that meets best practice. This nursing service allows service users to be discharged from hospital and continue treatment / care in the comfort of their own home.
We are interested in receiving applications from qualified Staff Nurses in the areas of community services, acute settings and public health who have relevant experience treating service users with a broad range of care needs including e.g. Dementia and who is comfortable providing/Overseeing such services in the community.
Please note that this role is subject to Garda Vetting and clearances.
Key Responsibilities:
· Provide clinical governance and oversight of the organisations care services, ensuring that service levels are consistent and maintained to the highest standard.
· Regular review and updating of all required policies and procedures within the company for the safe and effective care of the clients.
· Aid Hospital Discharges to ensure service users clinical needs are clearly understood and are capable of being provided at home.
· Input to service users Care Plans on all clinical matters.
· Attend relevant meetings with company and/or clients or commissioner of services and facilitate clear and precise clinical feedback.
· Review all adverse events and provide input to Care Manager re same.
· Train Carers both inhouse and in the field on clinical aspects of care plans.
· Provide support and advice to patient and family member's clients and multi-disciplinary team.
· To attend all company required meetings, training and other business in a prepared and punctual manner
* Have a clear understanding of HSE and HIQA governance.
* Complete care plans for clients care and update where necessary.
* Adapt a direct approach to clients and carers in reference to competency skills assessments with all Healthcare staff.
* Apply a person-cantered model of care to all clients.
Key Competencies & Required Criteria:
· Effective administrator, with a high level of attention to detail.
· Good time management skills.
· Excellent communication skills, ability to present to a multidisciplinary team.
· Ability to quickly build good rapport with new service users.
· Must have an active NMBI PIN on the general division of the register of nurses.
· 3 years plus experience in a health/social care or voluntary background.
· Experience working with people with Dementia, Stomas Care, Peg Feeding, Cathers and Neurogenic Bowel Care is desirable (Training can be provided).
· Quality standards and risk management experience would be desirable.
· Have a positive attitude and lead by example.
· Need to be able to work on your own initiative as well as part of a team.
· Full Driver's license & Access to Vehicle
· Currently Eligible to work in Ireland.
