Documentation and Reporting: Maintain accurate records including daily notes, incident reports, and updates to care plans.
Work Collaboratively: Liaise with families, multidisciplinary teams, healthcare providers, and community services to ensure coordinated care.
Health and Well being Monitoring: Support individuals with medication, appointments, and overall physical and mental health needs.
Promote Rights and Advocacy: Empower individuals to understand and exercise their rights, and advocate on their behalf when needed.
