As a highly skilled Clinical Nurse Specialist, you will work with patients aged 65+, providing mental health support and care. This involves assessing patient needs, developing personalized care plans, and offering education to patients and their families.
Key Responsibilities:
1. Conduct comprehensive assessments of patients' mental health needs, identifying key areas for improvement.
2. Develop and implement individualized care plans, tailored to each patient's unique requirements.
