Job Title: Care Management Professional
We are looking for an experienced and skilled individual to join our team as a Care Management Professional. In this role, you will be responsible for providing high-quality care and support to individuals with intellectual disabilities and complex health needs.
The ideal candidate will have up-to-date NMBI registration, a completed management course, and proficiency in the English language. They will also have experience working with people with intellectual disabilities, autism, and behaviors of concern.
You will be expected to lead by example, demonstrating excellent leadership skills and the ability to work independently. You will also need to possess proven managerial, organizational, and interpersonal skills.
In this role, you will work closely with families and multidisciplinary teams to provide person-centered care and support. You will be responsible for maintaining compliance with HIQA standards and regulations and have a detailed knowledge of rostering and leave planning for staff.
